For instance try C-c C-t C-t (rst-toc). AFAICS you tried one function. Indeed the one which I consider a PITA and from my point of view could be removed without anyone noticing. I'd say the rest of `rst.el` works like a charm and frankly through all the years I can't remember any such complaint.
