The easiest way to see how doxygen runs dot is to use the "-d ExtCmd" debug option when running doxygen from the command line. In short doxygen uses -Timap to generate a map file along with the image. Note that this is a server side image map, which doxygen then translates to a client side image map which is embedded in the HTML page. IIRC recent versions of dot also support generating the client side maps directly.
