document-dtd-entities

Hi Markus,

The document-dtd-entities script you copied into refdb/scripts is not 
required by refdb-ms at all.  It is a utility script that was designed 
to read through the associated library module and build a html help file 
of all citestylex elements and attributes.  I wrote it to aid me in 
understanding the style dtd.

At the moment it is broken, still looking for the 'refdb-ms.pl' library 
instead of the Refdb::Makestyle module.  I recommend removing it.
