I installed mysql5 from the official debian repository. The libdbi drivers were installed from the debianised packages on sourceforge. I had deb http://refdb.sourceforge.net/debian/cvs/ testing main deb http://refdb.sourceforge.net/debian/cvs/ unstable main in my sources.list and used apt-get. No tweaks were needed, as far as I remember. Though after creating a new reference database with refdba, I had to manually set the user privileges in mysql to be able to access the database from refdbc. refdb was configured with ./configure --sysconfdir=/etc --with-db-server=mysql then make and make install.
