Re: Adding SOAP header to response messages

When Axis2 initializes the SOAPEnvelope for outgoing messages, it
already creates an empty SOAPHeader object. Therefore you should use
SOAPEnvelope#getHeader() instead of
SOAPFactory#createSOAPHeader(SOAPEnvelope).
