New version of media types doc available

A new version [1] of the media types doc is checked in. This version 
incorporates all the resolutions adopted by the WSD WG at the Boston 
F2F. It also includes some ed changes. All the changes are documented in 
the change-log at the end of the doc.

Please note that the reference section has two updates:
1) RFC 2396 has been updated to point to the recently published RFC 3986 
(which obsoletes RFC 2396).
2) The references to XML Schema have been updated to point to XML schema 
2nd edition.

This fulfills all the AIs assigned to the media type doc editors.
