When using a PDF transcoder configuration like the following

  <fonts>
    <substitutions>
      <substitution>
        <from font-family="Helvetica"/>
        <to font-family="Courier"/>
      </substitution>
    </substitutions>
  </fonts>
</pdf-renderer>

then the "substitutions" configuration is currently ignored. The attached patch fixes this issue.
