Having trouble with using template

Hello.

Like jfor:stylesheet extension sample in jfor-0.7.1 
(simple-stylesheet-and-template.fo.xml + jfor-rtf-template-test.dot ) ,
I want to use Word template (*.dot)

When I use sample template, it works , but When I use my template, it doesn't.

I can generate RTF document, names are listed in the pull down style menu,
but not styled.

I make it with "Word 2000 Japanese edition".
Style name, template file name, file path etc.. are all in English.
I tested simple style ( i.e. just red font color )

Does anyone know what I am doing wrong?
If you don't mind, would you check my template?
