I think the jfor sample XSL-FO documents (available in the jfor samples 
subdirectory of the jfor source code download) should help you solve 
most or all of your problems.

So I suggest that you study the samples first, and come back to this 
list (one question per email is preferable) if you have more question 
afterwards.

There is one question that I can answer right away, though:

That's a common problem with java code that uses graphic libraries. 
Searching for the word "headless" in 
http://xml.apache.org/cocoon/installing/ should point you to the solution.
