I added <colspec/> elements to the tables in the DocBook XML source, ie. changed <informaltable> <tgroup cols="2"> <tbody> <row> <entry>IP-Address</entry> <entry>172.21.207.242</entry> </row> </tgroup> </informaltable> to <informaltable> <tgroup cols="2"> <colspec colnum="1" colwidth="5cm"/> <colspec colnum="2" colwidth="5cm"/> <tbody> <row> <entry>IP-Address</entry> <entry>172.21.207.242</entry> </row> </tgroup> </informaltable> and removed a <revhistory> element (which also creates a table in the FO) output, and ran the DocBook XSL style sheet to create XSL-FO.
