Clarification: startDTD, startEntity or both

I am confused about the meaning of the http://xml.org/sax/features/lexical-handler/parameter-entities feature: If it is turned on, should the DocType declaration trigger *both* events for the "[dtd]" entity? That is, startEntity() followed by startDTD()? (Likewise for endDTD(), endEntity() ) This seems like too much. If it is turned off, should startDTD(), endDTD() still be reported?
