Oh sorry, these things happen if you try to answer just from memory
without checking if it's correct ;-)

However, the point is that the skeleton file is not intended to be
applied directly. Rather use one of the customizations like
schematron-basic, -message, or -report. In your case you should
create your own customization that out outputs each message within
an XML element.

Yes, this works, but in order to benefit from future changes to the
skeleton you should put these things into a separate XSLT file.
(for example there is a already a version 1.6)
