When I changed keep-with-next.within-column="always" to 
keep-with-next.within-page="always", this solved the problem you describe.

This looks like a bug. If this is actually the case[*], we'll try to fix 
it in the next release (planned for this fall).

[*] It could also be a bug in MS-Word. That is, MS-Word may not honor 
some of Office Open XML 
(http://www.ecma-international.org/publications/standards/Ecma-376.htm) 
elements or attributes.
