Summary: Patches for XSDDoc

Initial Comment:
The following bugs were noticed:

a. Problems in the stylesheets when the XMLSchema
namespace was the default ie unprefixed. This caused
problems in that dead links were generated. The
attached patch fixes the ones observed.

b. Task.java was treating the filesets attribute in the
ant task incorrectly. A patch for that is included.

c. The documentation produced from the <xs:any> element
was incorrect due to inconsitencies between model.xsl and
xml2html.xsl. See  patch attached.

Patches in diff format. Should work with ant. Ive also
added more debugging information to Processor.java
