Summary: xsddoc: Display source file name

Initial Comment:
When a project uses multiple schema files it is useful to be able to quickly identify the file in which a documented component is defined. This patch adds the source file name to the "Source" heading.

If the source is a file and not a URL, the directory path is omitted.
