Your example is very helpful and I now am roundtripping unknown entities (I'll upload my example in case anyone cares, but it is just a slightly expanded version of what you sent). There is still a documentation bug, however. The sentence "Note that as the DOCDECL was ignored then during parsing may be fatal exception when undeclared entity is encountered!" on the web site is misleading. Something like "As the DOCDECL was ignored, references to entities cannot be expanded (they will produce a fatal error if you call next(), but will be reported as ENTITY_REF if you call nextToken() instead)." I'm marking this bug as reopened for the documentation issue.
