That would be "XXX" anyway (and "XXXX" is how Queenslanders spell
"beer").

The forward references are the subject of #77.

I was working on two-pass processing as a temporary solution for the
forward references problem, but I was tying myself in knots trying to
add an abstract supertype for all FOs that can have an ID, so I ended up
putting that in the 'idable' branch and reverting the trunk so I could
make xmlroff 0.6.1.

I wasn't aware of the repetition of page numbers for the index terms.
Please make a bug report.
