xmlroff status

I have done more work on xmlroff in the last couple of months [1] than in
the previous year.  I have merged the 'regions' branch into the trunk, but
there's still more work to be done on selecting page masters, markers, and
formatting page numbers before xmlroff gets completed headers and footers.
