What does 'smartctl -a /dev/sd<disk> say?  Any errors?

It could be that your hard drive is on it's way to RIP.   I had a disk
which used to loose the partition table.  With 'testdisk' I was able
to restore the part table but it happened randomly a couple of more
times.   Eventually, it had to be RMA'd.

I would suggest that, at a minimum, backup up your critical files from
this disk.
