I know this isn't a problem you've mentioned, but since Thunderbird stores its mail in Berkeley Mailbox Format, (choosing to separate its summary information in a parallel file) and I'm sure Evolution would do the same on principle, you should be able to simply copy your old mailbox files into Thunderbird's structure to have them appear within the program the next time it's opened. Thunderbird will build a summary file automatically, the first time it encounters them. P.S. I can't really help with the addresses, but since csv is only a kind of text, why not open the file and see if there was a particular reason it barfed - perhaps a comma in a recipient name that wasn't escaped, or something. Just an idea.
