$sudo port install libglade2

works for me. If you enable the libglade variant for php5-gtk, it will
pull in libglade2 automatically for you.

As for the demos, untar the PHP-GTK source package (from
/opt/local/var/macports/distfiles/php5-gtk) and you'll find them in there.
