I just was able to compile php 5.3.8 with --enable-intl.

My icu version is 4.2.1.It is installed in /usr.I did not needed the 
"--with-icu-dir" option.

My libxml2 version is 2.7.7 and i configured it with only "./configure 
--prefix=/usr".It did not need any patching.
