User notes become orphans when the manual pages change. For example, the
following note:

 http://pear.php.net/notes/admin/view-note.php?ajax=no&status=yes&noteId=7255

Is associated with:

 http://pear.php.net/manual/en/installation.cli.php

But that page moved.

There needs to be a method for managing orphan notes (to move them).
Here's an example orphan note system (for the PHP manual):
