PhD 0.4.5 released

Hello together,


PhD 0.4.5 has been released just before saturday, fixing the
show-stopping mediamanagerfilecopybug.

What else changed:
 Fix bug #47408: Same image directory used for each theme (Christian)
- Fix bug #47413: pear themes don't work when rendered together at
  once (Christian)
- Change copyright year to 2007-2009 (Christian)
- Fix PEAR chm manual title for french (Laurent)
- Fix PEAR chm navbar alignment (Laurent)

To upgrade from previous releases:
 pear upgrade doc.php.net/phd-beta

To install for the first time:
 pear channel-discover doc.php.net
 pear install doc.php.net/phd-beta
