I think the best way to get into the package is posting a mail to
pear-dev describing what your future plans with the Net_HL7 are.
Currently there are no open bugs, but I assume that you already have
plans with it.

If you like, you can also open bug and feature requests and attach
patches against latest SVN, or just fork the package on github and let
us review your changes.

After some successful patches, you will be given maintainer status.
