lexical structure changes - review?

Hi!

I've made some changes to lexical structure chapter here:
https://github.com/php/php-langspec/pull/69

Since it's a pretty large change it'd be nice if somebody would review
them, especially
the variable substitution part as I'm not sure everything there is 100%
correct.
