Recent Win32 build changes

I've converted all extensions to use COMPILE_DL_EXTNAME solely as shared
module/dll flag. All Win32 build files (.dsp and .mak) have been changed
accordingly. These changes should not cause any errors, however I cannot
verify that it works.
