Someone I work with (Judy) has put together some proposed "about PHP 
pages", see here: http://www.zapt.info/ianal/.
We tried to do this properly and send you a patch for the web pages - 
but in the end decided that this might be an easier way to review the 
suggestions. There is minimal new content in this, Judy has just 
collected together existing stuff and linked it, some of the older 
content in the FAQ has been removed and there are some minor tweaks to 
wording to make things clearer.

Ideally I think this should be easy to find from the main PHP page (so 
maybe a 'license' link needs to go there) and should also be linked from 
the CVS request form with a check box as you suggest.
