Modules are removed when their author delete them from the CPAN using
the PAUSE interface. But an archive is always kept in the BackPAN
    http://backpan.cpan.org/

You can use the module Parse::BACKPAN::Packages that Leon Brocard wrote
in order to recreate index and install old modules.

And for reviving old modules, you can try to contact the author, and if
he doesn't answer, just publish it again and ask to be transfered the
appropriate rights in PAUSE and RT.
