CGI.pm problem

I am trying run a simple cgi written in MacPerl that uses CGI.pm.  I receiv=
e
the following Diagnostic output when I run it.

Is my problem with my script, or my version of CGI.pm?
Is there a place for me to go learn what the Diagnostic outputs mean?
