With some hacks in place here is the patched version.

on windows you need to do:

go to http://github.com/kthakore/SDL_perl/tree/master 
click download, choose .tar.gz
save kthakore- .... .tar.gz somewhere

get to
http://cloud.github.com/downloads/kthakore/SDL_perl/libsdl-1.2.13-bin_20090825.ZIP
extract it on C:/strawberry/


on the cmd do 
pip kthakore- .... .tar.gz

... will be a long random variable!
