Fixed in trunk (r1933510) and 2.4 (r1933511).

Updated the mod_unique_id documentation to reflect the changes introduced in
2.4.29 (r1501827/r1812267):

- Encoding is now 160-bit (32-bit timestamp, 80-bit PRNG root, 16-bit counter,
32-bit thread index), producing 27 characters (was 144-bit/24 characters).
- Removed references to IP address and pid in the identifier composition.
- Removed the hostname/distinct-IP cluster requirement (no longer needed since
the PRNG root ensures uniqueness).
- Updated the encoding field ordering description.
- Added a note box explaining the 2.4.29 change.
