Fixed in trunk (r1933551) and 2.4 (r1933552).

Two improvements to IndexOptions documentation:
1. NameWidth now documents the minimum value of 5.
2. Added a note that the +/- incremental prefixes cannot be used with options
that take a value (IconWidth, IconHeight, NameWidth, DescriptionWidth).
