Strengthened the no-quotes warnings for Require ldap-group and
Require ldap-dn by promoting them from inline "Note:" text to
<note type="warning"> boxes, and explaining that these providers
do not support quoted values — unlike Require ldap-user and
Require ldap-attribute, which do.

We acknowledge this discrepancy between providers is confusing.
The differing behavior is a quirk of how each provider parses
its arguments, and ideally they would be consistent. The warnings
are now much more prominent so that users are less likely to be
caught out by it.

Patched in r1933732 (trunk), r1933733 (2.4).
