Documented AddOutputFilterByType context merging behavior:
- Directives within the same context are additive
- A definition in a more specific context (VirtualHost, Directory,
  Location) replaces — not adds to — definitions inherited from
  a broader context
- This is the same behavior as AddOutputFilter (now cross-referenced)
- Includes a server-vs-VirtualHost example showing the replacement

Patched in r1933734 (trunk), r1933735 (2.4).
