Fixed in r1933753 (trunk) and r1933754 (2.4.x).

Added a note to the ProxyPass connection pooling documentation
explaining that the acquire, ttl, min, smax, and hmax parameters
have no effect with the prefork MPM. Verified in proxy_util.c:
when max_threads <= 1 (prefork), the code zeros out min/smax/hmax
(L2309-2312), suppressing apr_reslist creation, so acquire (L2362)
and ttl (L2352) are never applied.
