Patched in r1933919 (trunk) and r1933920 (2.4). Rewrote the AccessFileName
section to clarify that AllowOverride must be explicitly enabled (addressing
the 2.4+ default concern), and updated the AllowOverride opening paragraph to
use "distributed configuration file" rather than assuming .htaccess.

Also removed the example of setting AccessFileName to a non-standard value.
Documentation should not only describe functionality but also recommend best
practice — suggesting that people rename their distributed configuration files
serves no purpose other than making troubleshooting harder for everyone
involved.
