Rewrote the AccessFileName section to clarify that AllowOverride must be explicitly enabled (addressing the 2.4+ default concern), and updated the AllowOverride opening paragraph to use "distributed configuration file" rather than assuming .htaccess. Also removed the example of setting AccessFileName to a non-standard value.
