Actually, yeah... That's right.... Mod_mp3 see's it has a large tcp window
size, and takes it...sends blocks of data down the pipe since you can handle
it... 

What happens is mod_mp3 sends the file in the correct way (mime-type,
shoutcast style protocol, etc..) but doesn't limit the bandwidth due to the
fact that simply isn't practical. That would have to involve some sort of
bitrate shaper thingy in mod_mp3, and since mod_mp3 is unaware of the
encoded bitrate of the file, it simply spews it all and dies as quickly as
possible. Now I will say that there is a way to use the LAME encoder in the
middle, but even then you will still get the mpeg data at the rate LAME can
spew it out, and I assume that is still faster than the logical bitrate of
the file (typically). 

It really does make sense if you think about it long enough.

In answer to your questions.

1.  Yes, it works one way, but not the other.  I'll
send you a private e-mail with the URL so you can see
it for yourself.

3.  Don't worry about this one.  The more I thought
about it, I think Ensim actually wraps the processes
with some kind of bandwidth monitoring whatsit, so I'm
pretty confident that the output from mod_mp3 will be
treated just like regular Apache http output.
