Does the apache process actually run under apache:apache, or is it 
configured to run for example as nobody:nogroup?

In some linux distributions, mod_perl is run as a separate apache process 
with different permissions. Particularly mandrake/mandriva is known for 
this.

Also check that apache has read/exec permissions to /usr, /usr/local and 
/usr/local/mod_survey.

If all else fails try doing "chmod -R 777 /usr/local/mod_survey/data" 
(this is just for debugging purposes, you'll not want the data folder 
world writable unless you are very sure there are no untrusted users on 
the system).

If this does not work either, I'll be interested in knowing distribution, 
apache version, mod_perl version and perl version.
