overhead?  not sure what you mean by that.  I admit that if you have
csound scores that are alerady doing what you need it might be a
little work to adapt them, though.

What [csound~] extern does is start csound application as a subprocess
so you can set up any command-line switch as you wish.  therefore, it
should be possible to route the midi via alsa virmidi module to and
from PD (I never tried it though.  
Admitedly it would be a bit of a hassle.  It'd be more
sensible to avoid using MIDI in csound and use it only in PD (in and
out if necessary) and generate csound score events within PD (that's
very easy to do).  Of course, if your csound orchestras rely on MIDI
already it'd be even more work to adapt them to work as I describe.
However, processing/generating MIDI in PD is very flexible and IMHO
pd's MIDI capabilities have more potential.  And then, you can send
audio in and out of [csound~] and do other things to it with pd.
