If you're comfortable with the command line: Beets (http://beets.io/) ticks
most - perhaps even all - of your boxes.
The people in the http://reddit.com/r/musichoarder/ subreddit might have
additional suggestions.
What are you using now? You can search http://alternativeto.net/ for
alternatives.
