One other possibility: “Bige” mentions “media player” as putting all the files in the playlist twice. I have seen, at least using fubar2000, situations where if the media library file types are set to include both .cue and .flac one ends up with an entry for both the ..flac file itself and for the cuesheet.

What are you seeing in the log file that indicates the disc was ripped twice? and if you look in the directory that holds the resulting FLAC files, do you see more than one copy of each track?
As an example, here is part of a log file showing the extraction results for one track of a recently-ripped CD:

EAC ripped this track, and then ran the FLAC compressor to compress it to FLAC. My copy of EAC has the box checked that tells it to delete the original wav after compressing, so my directory has only the filename ending in .flac. If EAC didn't delete the original wav, I'd see both the .flac and a .wav for the same track.
What are you seeing?
