I had this problem from time to time. I boiled it down to there being a loop or other audio file on a TDM?DAE track (eg a recycle or other loop I imported) that was a different resolution or bit depth than the session. Logic permits this but DAE doesn't. I also had trouble on my dual processor G4 that it (in os 10.3.4) would only function correctly in 44.1k and not 48k. So try toggling the resolution, quit logic restart and restart logic and reopen session. Also just to cover bases you have DAE and HTDM checked in the audio engine settings and coreaudio unchecked, right? Since in my experience this is likely a sample rate resolution issue, I am curious about the whole mindprint trio and CO2 routing. The CO2 seems to be a optical spidif to spidif coax converter, not an ADAT to spidif converter (2-2 channel vs 8-2 channel, get it). You are using this from the spidif out of the ADAT bridge and not in the ADAT/ optical connection? Get these out of the chain and see if things are working. Also try hooking up the Mindprint via analog connections. IMO the issue you have here is possibly the Spidif conversion, and or master slave settings on the Mindprint vs the protools rig, or the bit depth settings are mismatched and are making trouble. IMO the mindprint thing is not very good choice for this rig. A Mackie Onyx Firewire , MOTU 828 Mk II or III or RME Fireface 400 would give you a great input stage and allow you to use whichever you choose as a coreaudio interface, looping back into the ADAT bridge and allowing you to avoid using ESB and opening up some plug in chips in the process.
