It's easiest to just setup a small script/cron job that checks the last handler change entry in mantis_bug_history_table (for each open bug with a handler assigned) to see if it was 4-5 days ago. I imagine you'll want to avoid sending duplicate emails (or only send duplicates once a day) which is something that is very easy to do with a nightly cron job.
