Tracking different parts of a document

One of the requirements that we have for a Document Management Solution
is as follows:
 
A user can create a word docuemnt with different items. Each item would
typically be a headline and paragraph. We need to be able to track each
of these items individually. Each item would have associated metadata
and versions. So for example, we can give a search for all the documents
which have a specific item. At the same time, we want to do document
management for the doc also, which means doing things like workflows,
versioning etc.
 
Giving a form based solution is NOT an approach that we are looking at.
MS Word has to be the primary interface. So, when a user updates a
particular item in the doc, it should get syncronized with the database
or repository.
 
The client wants to use docuemntum and Oracle to implement this.
 
Would appreciate any pointers for this.
