You might not need a CMS for this.  Just a simple databased web  
application should do.  There are a number of frameworks out there that  
will get you most of the way there.  JBoss Seam (Java) and Symfony (PHP)  
might be worth considering.  If you want a little more CMS functionality,  
you could look at modx, which is a web application framework with some  
additional content management capabilities.  All of these options have  
nice AJAX component integrations that might be useful for data validation  
and lookups to help with usability.
