The latter is clearly an error in the code. Whenever you go from form page
to form page Xaraya keeps track of where you came from, as it were, to make
sure the information passed is from the expected flow of pages.

As to the former, I'd guess the module's init.php has some error in the code
that creates the module's db tables.

Flag 'em both as bugs. Thanks.
