Did you copy user files from a previous installation instead of using the official method of migration from the Toad initial setup wizard? You can confirm that Toad is using the new user files if the files in your <AppData>\RuleSets folder have a modified date of August 17, 2011. If they have a modified date of March 29, 2006 then you have old user files and you should follow Brad's suggestion of removing your appdata folder and starting Toad. We've seen this happen when Toad is installed over top of an older Toad version, or 2 versions of Toad are pointing to the same application data folder. Close Toad. Go to options > General and locate your application data folder. Remove the folder named after the version of Toad (11.1). Run Toad and when prompted you can then let Toad migrate your user files over from a previous version. If you actually installed Toad 11.1 into an existing Toad folder, I suggest you uninstall all instances of Toad on your machine and re-install into their own, fresh directories.
