I have seen this before... Some times it is a memory issue. 


Another user had the problem every time she tried to execute a script.......one
try one

...on try two it would be fine... we eventually re-installed toad on that one

We tested Toad with different Oracle clients (10.2.020 and 11.2.010), but same result.
