I have seen this exact same error occasionally when trying to send mail 
through my stunnel'd qmail-smtpd + smtp auth server using checkpassword 0.90 
when using KMail 1.6.1 as the client (other clients don't show the error, 
either because they don't show server errors directly or just retry I'm not 
sure - I don't see how the client matters).  I simply retransmit the message, 
and it works fine.

It's very strange because it is so sporatic, and I didn't know that 
checkpassword might be to blame until I saw this post.
